The challenge

To meet their population health goals, Gundersen Health System (GHS) needed to understand their active patient population and scale patient outreach. Over 10% of patients on primary care panels had not seen their primary care physician (PCP) within the last two years, and automated outreach through the patient portal in the EHR was unidirectional and ineffective.

The solution

GHS worked with Olive first to implement a range of solutions that utilized an evidence-base algorithm to sort patients by demographics and engagement and then to confirm the data’s accuracy and quality. Today, the automation runs weekly, evaluating all new visits, reaching out to new patients and automatically removing patients as necessary from the panel.
